Registering a team for the spring season

Existing Teams that were registered in the fall of 2007

Teams that were registered in the fall of 2007, that don't have any changes to their roster, will only need the following items to register for the spring 2008 season. (1) A Team Entry Form that will be provided at registration and (2) Your Team Registration Fee (U6 is $27.00 per player and U8 and older are $40.00 per player). If you are adding any new players to your fall roster they will need, a CBYSA Membership Form, a Parents Code of Conduct Form, a Birth Certificate and their Registration Fee. If you're removing players, Player Cards need to be turned back in to the Registrar.

New Teams will need these items for registration

  • Team Roster Form

  • Uniform Order Form

  • CBYSA Membership Forms (one for each player)

  • Parents Code of Conduct Form (one for each player)

  • Volunteer Disclosure Form for each coach (to be completed on-line)

  • Entry Form (provided at the registration meeting)

  • Birth Certificate or Federal/State issued document for each player

  • Registration Fees (U6 is $27.00 per player and U8 and older are $40.00 per player)

This document will be collected by the coach prior to season registration dates: Any state/federal issued document - e.g. Birth Certificate, Passport, Adoption papers or Foster Papers

If you need to order new uniforms this season please make your selection from the following website (www.scoresports.com) and place the information on the Uniform Order Form. You can make any jersey selection $16.95 or under and the entire uniform price will be $18.00.
