The deadline for Player Registration for the Spring 2008 season was January 11, 2008.

TEAM REGISTRATION FORMS

  • Team Entry Form will be provided at registration

Team Roster Form
Three copies of this form are required for registration night
Uniform Order Form
Teams ordering new uniforms must complete this form and turn it in to Pete Petersen as soon as possible. Petes phone number is 323-3339.
CBYSA MEMBERSHIP FORM - every parent must complete
CBYSA Membership Form - Must be completed by each player and turned in at registration.
Parents Code of Conduct
This form must be completed by each parent and turned in at registration
Coaches/Assistant Coaches Volunteer Disclosure Form

    Player Placement Form (Transfer Form)
    For players that are requesting placement on a different team.
    Multiple rostering Form
    This form is used to roster a player with two teams.

    This form is for parents seeking financial assistance for their children.

    Medical Release Form
    This form is for Tournament use.
    Fall Invitational Application
    This form is to register your team for the tournament.
