All forms needed for registration on July 21st are available here.
The forms have been set up as templates. You can use the tab key to move through the fields and type the information and use your mouse to click on drop-downs or check boxes. You can save a copy to your hard-drive if you wish. Print the number of copies requested and bring them to registration on July 21st. If you'd rather, you can print the forms and fill them out. For each form, the coach should keep a copy. The other copy(s) will be turned in at registration. Team Entry Form - Print 2 copies
Enters your team for the season; includes your scheduling conflicts (if any). Team Roster Form - Print 3 copies
Three copies of this form are required for registration. CBYSA Membership Form - Print 2 copies
Must be completed by each parent and turned in at registration. For the Fall Season, this form must be completed for ALL Players. Don't forget to get a certificate of birth for new players. Parents Code of Conduct - Print 2 copies
