    Team played Fall 2011 and has NO changes

    Team Entry Form and Fees for each player

    Team played Fall 2011 and has only a coaching change

    Team Entry Form and Fees for each player; Coaches Disclosure form and Code of Conduct

    Team played Fall 2011 and has player changes

    Team Entry Form, Updated Team Roster, Fees for each player. Membership form, Parent code of conduct, certificate of birth for each new player. Player placement form if player is transferring or dropping from the team for other reasons. Uniform Order form if uniforms are needed.

    Required for Team Registration

    The following forms are required as noted for Spring Team Registration.

    Team Entry

    Enters your team for the season; includes any scheduling conflicts.

    Team Roster

    Required. Can reuse roster from Fall 2011.

    Player Membership

    Needed for each player NEW to your team.

    Certificate of Birth

    Needed for each player NEW to CBYSA.

    Parent Code of Conduct

    Needed for each player NEW to your team.

    Volunteer Disclosure Form

    You will register online with Iowa Soccer Association. Must be completed by NEW Coaches and NEW Assistant Coaches. See link on the right.

    State Requirement for Coaches Hard Pass

    NEW COACHES ONLY---to obtain your Coaches Hard Pass a photo in .jpg form must be sent to Helena Young, CBYSA Registrar, at nene4046@yahoo.com
